What we collect. If you use our contact form, we receive the name, email address and message you choose to send. If you call, text or email, we receive your contact details and whatever you share. Our website is hosted by Squarespace, which collects standard analytics (pages visited, device and browser type, approximate location) and may use cookies.
How we use it. Only to respond to your inquiry, schedule and provide services, run and improve this website, and meet legal obligations. We do not sell your information or use it for advertising. We share it only with service providers who help us run the website and practice, such as Squarespace and our email provider, and when required by law.
Please keep health details out. The contact form, email and text are not secure channels. Please do not send health or clinical information that way; we will talk about those details by phone or in session.
Your choices. You may ask what information we received through this website, or ask us to delete it, by contacting us. Treatment records are kept as required by Michigan law. This website is intended for adults. We may update this policy; the effective date above shows when it last changed.
Notice of Privacy Practices
This notice describes how mental health information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Lemon Tree Therapy is required by HIPAA and Michigan law to protect the privacy of your health information, to give you this notice, and to follow the notice currently in effect.
Without your written authorization, we may use and disclose information for treatment (including consulting with your other providers, with your permission where required); payment (such as creating a superbill for out-of-network reimbursement); and health care operations (such as record keeping and clinical supervision). Lemon Tree Therapy practices under clinical supervision where required. We may also disclose information when the law requires it (such as a valid court order or subpoena), to report suspected abuse or neglect of a child or vulnerable adult, to prevent a serious and imminent threat to health or safety, to a licensing or oversight agency, and to emergency responders in an emergency.
With your written authorization, we make any other disclosure, such as sending records to an attorney or employer. You may revoke an authorization in writing at any time. Psychotherapy notes receive extra protection. We do not sell your information or use it for marketing.
Your rights. You may ask to see or get a copy of your records (we may respond within 30 days and may charge a reasonable copying fee); ask us to correct information; ask for a list of certain disclosures; ask us to limit how we use or share your information (including, if you pay in full out of pocket, not sharing with your health plan); ask us to contact you in a particular way; get a paper copy of this notice; and be told if your information is involved in a breach.
Telehealth and electronic communication. We use a secure, HIPAA-compliant video platform for online sessions. Email and text are not fully secure, so please use them only for scheduling and general logistics.

Your Right to a Good Faith Estimate
Under the federal No Surprises Act, you have the right to receive a Good Faith Estimate of the expected charges for your care if you do not have insurance or choose not to use it. You can ask for an estimate at any time, before scheduling or during care.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you may have the right to dispute it. You must start the dispute process within 120 calendar days of the date on the bill. Keep a copy of your estimate and your bill. For more information, visit www.cms.gov/nosurprises or call 1-800-985-3059. To request an estimate, call 231-675-6245.
